The approval process is as follows:

• On receipt of form MHA1, the form is checked for completeness, if there are questions not answered correctly the form will be returned, with an explanatory letter.

• Two references are sought.

• On receipt form MHA1, your CV, references and supervised assessments (if required) they are copied and posted to South West Section 12(2) Panel Members for their opinion as to your suitability for approval.

• On receipt of the panel members opinion, you will be notified with one of the following decisions:

• You have met all the criteria requirements and are now Section 12(2) approved.

• Your application has been successful and you will be Section 12(2) approved after your attendance at the Section 12(2) Induction Course. (NB: under no circumstances will approval be granted before attendance at the induction course)

• The panel members have requested that your application be discussed at the next full meeting of the South West Section 12(2) Panel, the date of that meeting will be given.

• The panel do not consider that you meet the requirements for approval, their reasoning will be given, together with advice as to what you need to do to fulfil the criteria.

Please note that the paperwork process will take upwards of 6 weeks.

APPLICATION FORMS FOR INITIAL AND REAPPROVAL UNDER SECTION 12 OR APPROVED CLINICIAN

The two application forms are suitable for doctors applying for initial Section 12 approval or Section 12 reapproval in the South West.

They are not the correct forms for those wishing to apply for Approved Clinician Approval or renewal. Please see the Approved Clinician section on this site.

SUPERVISED ASSESSMENTS

Doctors applying for approval under criterion B, C or D will need to undertaken two Mental Health Act assessments supervised by a Section 12(2) doctor. Below you will find guidance and the supervised assessment forms which need to be completed and returned to Hilary Eagles if applying for approval in the South West.

CHANGE OF CIRCUMSTANCES

It is important to note that it is the responsibility of the individual practitioner to ensure that proper authorisation is maintained. It is however our policy to remind approved practitioners that their approval is about to expire and, if appropriate, invite them to apply for renewal. However, we can only issue such a reminder if the practitioner keeps us informed of his/her address.

  • CHANGE OF CIRCUMSTANCES FORM
    It is of utmost importance that you inform the regional section 12 office that you are working in their area, to ensure the SHA statutory requirements are met, that AMPHs can call on you and that your payments are processed.
